Schedule of works for insulated foundations

I need to produce a schedule of works for my insulated foundations, has anyone got anything they would be willing to share so that I could save time by plagiarising it?

Are these raft foundations?

Raft foundations designed by Hilliard Tanner, being installed by myself and a local foundations guy who's never seen this style of foundation before. The foundations are essentially three intersecting rectangles, the largest of which is set into a hill side and will have reinforced retaining walls on three sides.
