Making holes in insulated panels

From my understanding the insulated panels like kingspan quadcore work as they are enclosed tightly sealed panels. How would ventilation/ducting be fitted to a building wall made from quadcore?

Looking up service void it’s a layer between the wall and finishing. 
 

Im trying to understand a hvac or similar duct going through the interior layers through the external to vent out or take in.
